Diagnosis:

1. TIA

2. New AF

​

Plan:

1. Commenced new Apixaban 5mg BD

2. Commenced atorvastatin 20mg OD

3. No driving 1 month

 

 

Dear GP Colleague,

​

It was a pleasure to review Jo in TIA clinic today. Joe had presented following a 5 minute episode of left face, arm and leg weakness on 13th October. Symptoms fully resolved.

​

Past medical history includes type II diabetes, Hypertension, osteoporosis and migraine. Jo lives fully independently and drives. There is no smoking history.

​

Examination today was normal. Blood pressure 125/80 in clinic.

​

MRI head today was normal. Carotid dopplers showed 10-49% stenosis in both internal carotid arteries.

​

ECG today showed new atrial fibrillation rate 88.

​

Plan:

​

Jo has had a right hemispheric TIA in the context of new AF.  I have started anticoagulation today with Apixaban. 

​

I have given advice to reducing stroke risk in the future and told Jo to refrain from driving for 1 month.

​
